If you disagree with the ministry’s reconsideration decision:

Fill out a Notice of Appeal (pdf) form. A Notice of Appeal (pdf) form should be with your reconsideration decision. You can also download a Notice of Appeal (pdf) form.
You can send more information to the Tribunal after you send us your Notice of Appeal (pdf) form.
